By completing this form you hereby agree to participate as a volunteer with Citizens Caring For Clearlake (“the Organization”). In consideration for allowing me to participate in volunteer activities, I agree to the following terms and conditions:

1. Assumption of Risk: I understand and acknowledge that volunteering may involve certain risks and hazards, including but not limited to physical injury, illness, or property damage. I accept full responsibility for any such risks and hazards.

2. Release of Liability: I release and discharge the Organization, its directors, officers, employees, and volunteers from any and all liability, claims, demands, actions, or causes of action arising out of my participation in volunteer activities with the Organization.

3. Medical Treatment Authorization: In the event of any injury or medical emergency during my participation in volunteer activities, I authorize the Organization to obtain medical treatment for me as deemed necessary by medical professionals.

4. Photographic Release: I grant the organization the right to take photographs or videos of me during volunteer activities for promotional or informational purposes. I understand that my name may be used in connection with such photographs or videos.

5. Parent/Guardian Agreement (For Minors): I certify that I am the parent or legal guardian of the below named minor and I have read and understood the terms of this Volunteer Release Waiver. I agree to indemnify and hold harmless the Organization from any claims or liabilities arising out of my child's participation in volunteer activities.

6. Consent for Minor: By signing below, I consent to my child's participation in volunteer activities with the Organization and agree to be bound by the terms of this Volunteer Release Waiver on behalf of myself and my child.
